Our services include primary and pediatric care, dental care, behavioral and SUD care, supportive housing, and community re-entry services, to name a few. The Case Manager provides support and advocacy to clients and their families within the community to educate and promote healthy lifestyles and preventing reoccurring hospitalizations by monitoring, assessing, advocating, supporting and empowering members to improve their quality of life through maintaining stability in the community.
